As the number of medications used increases, so does the risk for adverse drug reactions, drug interactions, and reduced adherence to the prescribed regimen.

            What is  Polypharmacy?
Medication Therapy Management

Polypharmacy is the use of multiple drugs, or more than are medically necessary, and is a growing concern for older adults.

Medications that are not indicated, not effective, or constitute a therapeutic duplication would be considered polypharmacy.

The burden of taking multiple medications has been associated with greater health care costs and an increased risk of adverse drug events (ADEs), drug-interactions, medication non-adherence, reduced functional capacity and multiple geriatric syndromes.
